Learnings and results from a nonpartisan, education and awareness campaign designed to engage seniors, advocates and policymakers to prioritize the needs of California’s aging population. This lively panel discussion will explore how citizens and organizations alike can drive efforts as we move toward developing a vision and long-term investment plan for aging that addresses the increasing need for senior-related healthcare and other supportive services, using California as a case study. This campaign is part of a broader strategy between West Health and The SCAN Foundation to elevate older adult issues, break through the media clutter and ensure state policymakers prioritize these issues.

With an increased recognition of the impact of social determinants on health, there is a growing demand for holistic care models that seamlessly link clinical care and community supports to better address patient needs. Learn how a geriatric-focused academic health center developed and tested a standardized process to identify unmet social needs among senior patients and connect them to community supports using a community-based social worker and electronic care coordination platform.

Description
Family caregivers are more likely to have frequent interactions with the health care system when caring for their loved one. As such, they have strong opinions and perceptions of how well the system is responding to the needs of seniors. In this session, we present results of a national survey that assessed perceptions of the health care system and is the extent to which perceptions are influenced by age and caregiver status. Additionally, we will discuss West Health Institute's research efforts to identify and support the family caregiver.

Family caregivers are a vulnerable population relative to non-caregivers and are more pessimistic about healthcare in the US. They report more difficulties accessing care and believed innovative approaches such as in-home care and telemedicine would be helpful.
