Using Technology to Facilitate Coordinated Care Across Clinical and Community Service Settings (ASA19 workshop)

With increased recognition of the impact of social determinants on health, there is a growing demand for person-centered care that seamlessly links clinical care and community supports to better address senior patients’ needs. As part of a collaborative research project, the Gary and Mary West Health Institute (WHI), the University of California, Irvine Senior Health Center, and SeniorServ developed and tested an innovative care model, the 36O° Caregiving Solution, that enables seniors to maintain their independence and successfully age in place. In this presentation, we provide an overview of the project, introduce tools and processes to identify and address social needs, and discuss an innovative electronic care coordination platform that links patients with community supports.
