We are exploring opportunities to advance solutions for our nation’s seniors living with chronic or serious illnesses.

The Opportunity

It is estimated that over 90% of older adults suffer from some form of chronic disease, the most common being cancer, heart disease, stroke, and Type 2 diabetes. More than half of older adults have more than one chronic condition, and almost 15% are diabetic.

Our Goal

By exploring solutions that improve care transitions, produce better patient outcomes and increase efficiency, West Health can help more people living with chronic conditions receive care in their own homes on their own terms, enabling them to age successfully, with dignity and independence

Gary and Mary West Senior Dental Center

Launched in 2016 with a capital grant from the Gary and Mary West Foundation, the Senior Dental Center links high-quality and affordable oral healthcare with a suite of nutrition, case management and wellness services offered through Serving Seniors’ Gary and Mary West Senior Wellness Center, creating a one-of-a-kind, integrated community-based system of care for older adults.
